Video-assisted thoracoscopic surgery for primary hyperparathyroidism with ectopic parathyroid adenoma in thymus

Introduction. Primary hyperparathyroidism is rare pathology in children (2?5:100,000). In more than 85% of patients, a single adenoma present, and its extirpation usually the only treatment patient requires. approximately 15?80% cases, ectopic mediastinal parathyroid tissue can be found inside thymus. Case outline. Our was 13-year-old boy, who presented with multiple bone fractures previous period time, fatigue. Parathyroid hormone levels preoperatively were extremely high (1320 pg/ml ? 19 times higher normal). Serum calcium also elevated (total 3.55 mmol/l; ionized 1.41 mmol/l). He examined diagnosed as primary by pediatric endocrinologist. Imaging procedures for preoperative localization adenomas done (99mTc sestamibi scintigraphy magnetic resonance imaging suggested adenoma). The underwent video-assisted thoracoscopic surgery procedure. After exploration mediastinum chest, no found, so total thymectomy performed. Final pathological section confirmed Conclusion. We believe that if during surgical mediastinum, child detected anterior recommendation to think about possible intrathymic consider removing Greater sample size necessary reliability this statement.
